Letter

I{m going for exchange to berlin and they asked me to send a letter in english in which my professor allow me to go there. My proffesor doesn{t know english so he asked me to write it and he signs it, but I don{t know how to write it. How do I start it? It must be formal or informal? Do you have an example of this type of letter? Thank you a lot!
