Let the inmate be/heaving

Hi

In prison one of the guards hit the prisoner in the gut with a billy club. The inmate threw up and fell to the floor.

Then the guard: paused (he was talking all the time) to let the inmate be wrenched back to his feet, still heaving and gasping for air.

Does it mean someone helped the inmate to get back on his feet or he did it himself?

Does "heaving" here mean that he was "breathing heavily"?
