Which of the following sentences are correct?

a)"She is writing down his address lest she should forget it."
b)"She is writing down his address lest she forget it."
c)"She is writing down his address lest she forgets it."

Also, can anyone explain what tense of the verb should be used with the word 'lest?'

b)"She is writing down his address lest she forget it." (This is the best.)

If c) is the right answer, then it means that in the above sentence, 'sway' should be replaced by 'sways?'

But ( c ) is not the correct answer. If it were, then "sway" would be "sways."
