Lesson plan wording

Hi. Please help. Let us say there is a math teacher and is working on his simple lesson plan. I think we normally see headings like "Topic," "Type of Materials" and "Time" on the plan sheet. Do you think he should include the definite article when writing the name of the worksheet like "The Multiplication Worksheet," possibly under the heading of "Type of Materials"?

Topic - Simple Multiplication

Type of Materials - The Multiplication Worksheet or Multiplication Worksheet?

Time - 45 min.

How about if a worksheet has a made-up name like "Where can I buy it?" as its worksheet title (or heading) on the sheet? Should the name below be with or without the definite article? Thank you in advance.
