Leave application for wife delivery (Major Surgery)

Dear Sir,

I want to ask for a 10 days leave from my company for my wife delivery, Her due date is on 20/01/2018 and it is not a normal delivery, As her doctor prescribed she should go under a major surgical process.

Hence, Our first baby was delivered through the same procedure her doctor prescribed that she must go under the same procedure.

Please advise.
