Kyiv or Kiev, Belarus or Byelorussia...

After the USSR stopped to exist I noticed that some names of cities or countries changed. Who regulates this in English? 20 years ago 'Kiev' was the right form. Now I see 'Kyiv' everywhere. We officially say 'Moldavia' in Russia, (even in news programs on state TV) but in English I might see 'Moldova' .
Which forms are right ones and who regulate all this?

Kyiv = "????" in Ukrainian.
Kiev = "????" in Russian.

After USSR breakup the official language of Ukraine is Ukrainian only. That's why Kyiv is more accurate.

I don't think there is any official regulation. Lately, people probably use Google to check spellings, so whichever one is used most often becomes the "official" spelling.
