Know and happen to know

Medicine has gone underground.

People prefer not to go to hospitals if they are ill, but furtively ask their friends if they happen to know a good doctor.



I would write 'if you know a good doctor'. What makes the difference between 'if you happen to know a good doctor' and ' if you know a good doctor'?
