Just a matter of perspective: a or the shirt that he wore

Hi. Please help. What is the difference? I think, for sentence 1, they mean almost the same, but for sentence 2 there might be a difference in that the definite article points to the specific man, whereas the indefinite article points to one out of many.

1. He has on a/the shirt that he wore many days.

2. He saw a/the man who was sick and got healed.
