"It is I who is" or "It is I who am"

In standard written English, which is correct?

It is I who am (responsible for everyhting here).

or

It is I who is (responsible for everything here).

Is this also common in spoken English? or do Americans say "It is me who..."
please include rules for the placement of commas if there are.
