Issue of New Driving Licence

Dear Sir,

This is to your kind notice that I Mahesh Kumar lost my Original Driving Licence and would like to issue me new driving licence as early as possible. I am enclosed herewith a copy of FIR for your kind perusal. I request you to kindly issue new driving licence as early as possible.
