Issue and if clause

Hi, Everyone!!

In the passage below, I have two questions unsolved.

Not only is the idea being expressed that it’s not essential for there to be a physical healing if there is a mental regeneration around the issue embodied in the disease or the injury, it is becoming a strong belief that physical healing should not be associated in your minds with what A Course In Miracles is about.

1. The if clause.
Is it to be understood as, "if we want to make a ment al regeneration happen" or simply, "if the mental is regenerated"?

2. What is "the issue"? Is it something some kind of subject or topic to talk about or is it something physical?
