Is written

The severity and interaction of these ailments has resulted in my having a learning contract on which mental health problems is written.

i don't like this sentence. to me it sounds unnatural and clumsy...

Yes. Things go pear-shaped after "resulted in". The part about writing mental problems on a contract is too impenetrable a metaphor to be useful.
