Is this sentence grammatically correct?

His doctor has extended his medical leave for 7 days which end on 21.06.2021. As per his doctor’s advice, he requires another more 7 days for postoperative care.

It is ambiguous. The 7 days end, but does that mean that the date you name is the last day or will the 7 have ended on that date? Also, are we talking about two sets of seven days,
