Thanks for answering. So should the "to" be removed before the word kindly in the sentence, since you have highlighted it?

Here by "sender" I am actually referring to the poster - the one sent the package to me.

ScottishFrank is a native speaker and has given you good advice. Yes, delete 'to' if you are keeping 'that'. If you want a more concise version, here's one. I presume that the letter is addressed to the USPS, so the first part is useless verbiage, for instance, and 'non-delivery' cannot be proven, of course. You have other redundancies:

Please provide me proof of delivery or other email confirmation that a packet was delivered to that address and recipient by that sender.

Is this sentence correct?
