Is this paragraph correctly composed?

We had a couple of incident reports. Medical reported an ergonomic problem, WIC reported a person slipping on a wet floor, DCHS had a person who tripped over a box.

Two suggestions.
Couple means two or, more loosely, few. Better to be accurate in this case. You have three incidents, say three (or say, a few).
Don't use commas to join independent clauses. Either make each incident a sentence or use a semicolon to show that they are linked.

Here is how I would write it:

"We had three incident report
