Is this okay?

What happened to your face?

Oh I just fell down some stairs last week.

You fell down some stairs? How could it go so wrong? Were you drunk?

Is it okay to say "How could it go so wrong?"? And is it the same as "How did it go so wrong?"?

Not just in reference to going down the stairs, no. (The question could be appropriate if going down the stairs was incidental to executing some larger plan. "it" would then refer to this plan. However, this doesn't seem to be the case in your dialogue.)
