Is this formal? Can you help me write it better?

Hi,

Is this formal and polite when addressing your higher authority?

My email contents goes as below...

"As per my call with you, can you please help me to get myself formally relieved from all my duties in ABC Inc by approving on my reliving through this email to the respective persons CC'ed in here?

It is far from clear what you want :-( - but here goes "As per my telephone call to you, could you please help me to get myself formally relieved from all my duties in ABC Inc by approving of my being relieved by email to the respective persons CC'ed herein?
