is this correct?

Hi Guys, please can you let me know if the following sentence is correct please?

'Had the involved passenger made me aware about the incident and the resulting injury, I would have had followed the protocol and reported it to you.'

if not, please can you suggest something similar? English is not my first language and I would very much appreciate your help.
