Is this always necessary to start a certain sentence with a capital letter?

Is this always necessary to start a certain sentence with a capital letter? I don't think so but I couldn't come up with any example right now. What do you say?

I don't start with a capital when the first word is part of an italicized expression that I am discussing.

-- What is the difference between in time and on time?
-- in time means ... , and on time means ....

Other than that, I capitalize the first word of every sentence.
