I think only "would you" is suitable for that blank, but the answer is "do you".

So do I since it's an imperative. If it were "You have a piece of chocolate, ...?", all except "would you" would work.

Oh thank you, my book noted that sentence means "would you like to have a piece chocolate?" so there is "do you". @@
