Is the gerund in this sentence..

This form in which I explain the difficulties leading to its relevance should provide you with evidence arranged chronologically from: my tutor, my doctor and their adviser. It should warrant an overturn of the 40% module cap placed on six modules, in which there are 12 assignments, that would otherwise remain on my returning to the course.

My problem is the second sentence where it begins ,'in which there are ...'. Sentence flow is disrupted but I think this is the most concise way of phrasing these two sentences, right?
