Is sentence correct?

Our church will not be able to achieve the objective of having a total of 5000 members if we do not make sufficient efforts or take any constructive actions to realise the target.

Are the words in bold fine? Are 'a total of' and 'constructive' necessary? Please correct any other errors in the sentence.

It should be 'Is this sentence correct?' instead of 'Is sentence correct?'

I think for title, leaving out 'this' is fine. I don't know whether other members agree with me.
