Is it wordy and confusing to use (that) in this sentence or it is formal

Her world fell apart the moment (that) she had heard (that) they refused her application.

Some people told me that it is wordy and confusing to use that while books and grammarian say the opposite. I read that it's grammatically incorrect to use two independent sentences without a conjunction or punctuation.

My second question is: Do you place the past perfect in the adjective clause or the noun clause? Like in the following example: Do you say "Her world fell apart the moment that she had heard that they refused her application." or "Her world fell apart the moment that she heard that they had refused her application."

Oh, and another question because I got confused: When you have an adjective clause like in "she heard," do you place the adjective clause without a conjunction in formal writing?
