Is it correct to say to a person

I notice more and more people on the phone saying. 'Your name was?'

I usually respond by saying' My name still is'

My question is if it is correct to state 'Your name was?'

In the context of a phone call where the caller has already stated his name and the person asking the question didn't write it down (thus having to ask again) this is fine. It acknowledges to the caller that the person asking made a mistake in not noting his name the first time it was stated.

Ray's answer is correct. I would rather the person say something like, "May I have your name again, please".

When people speak, they often speak in fragments rather than full sentences.
I think the situation is that in the early part of the conversation, names were exchanged. Later on, one person wants to have the other repeat his name.
The full question would be "I beg your pardon, but what did you say your name was?" It is not asking for a former name, but reported speech - what had been said
