A slight rewording would help. However, I am not sure what you mean by the last phrase "by the account holder."
Is this what you meant to say?

Kindly discard the account number listed below as it was cancelled by the account holder due to a signature mismatch.

Or do you want to say that it was cancelled because the signature did not match the one you had
