ironic, best

I questioned about 'abstemious' before.

The question of this time is, is the second sentence ironic?

‘We only had a bottle.’ ‘Very abstemious of you.’

And, what does 'best' here mean?

The patients are best advised to be totally abstinent from alcohol.
