I would like to know the result of an interview that I had recently. But I'm very much concerned about the words or any mistakes in the mail. I want to send a not very formal mail. Please correct my template following here with proper words or in proper way.

I wanted to follow up in regards to the interview on dd/mm//yy. I have been to told to fill up an interview record form and I will be updated by the HR. Please let me know the status at your earliest convenience.
