Which of the above is correct?
Does the first one mean that there are more than one instructions in the manual?
Does the second one mean there is only one instruction in the manual, which is strange to me?

Neither could be considered in correct. There is no difference in meaning. I think the first choice is more common when written, while we drop the s when speaking.
