As far as I can tell with no more context, it just means chief (i.e. main) instigator. In this meaning it is not slang or specifically AmE. It would help if you could post the whole sentence.

I would typically hear this phrase when a person is talking about the #1 troublemaker. The person who starts sh*t is the instigator-in-chief, and they're not just a regular instigator: they also get all their troublemaking friends to come along. They're head of instigating things, like starting fights, and getting their buddies to jump in on it.
