Information on this and information on that is or are.

Hi, I've been having a debate at work and we cannot prove who is right. In the following sentence, would we say is or are?

"Information on ordering forms as well as contact information for the Centre is or are accessible from the following URL: . . ."

The second "information" in the sentence would always be preceded by a word, since otherwise it would just be "as well as on the Centre" in which case we wouldnt be having this debate. Thanks for any help given!
