hi there, i just wondering how to prepare an incident report and send thru email.
The incident was : while we were giving a training to our employees in the theater, a piece of ceiling tile has fallen down to the stage. By God's grace, no body got injured. The theater was full with more than 190 employees.
Please help me to prepare a standard reoprt.
