incident report letter

Please help me with this letter. This is my first time writing an incident report so im not sure if my format is correct. thanks in advance & Godbless..

This account is regarding the incident that happened last January 9, 2016

I am the morning charge nurse. when this incident occured, ms.ruby and mr. kevs was my co nurses and mr muya was our nursing attendant. at around 11:30 am after we transferred all postoperative patient we asked mr. muya to deliver the soiled linens to the linen department. At 1:45pm ms.ruby saw linens was not delivered so that we endorsed it to the afternoon shift nurse ms. angel and nursing attendant mr vic. but they also failed to do it because mr.vic said that linen department is already close at that time.

You have many subject-verb agreement issues. Plural subjects get plural verbs.
