incentive approval

Dear Sir,
I would like to request to approve my incentive for the month of Feb 2015. Till dec this were approved by Mr. Mathew. While I have been transfer to logistics dept kindly approve my incentive for Feb and March( Jan I was on vacation) and please forward to accounts.

my incentive - do you perhaps mean my bonus?

ie a wage payment whereby a worker is paid an additional amount for accomplishing more than a specified measure of work

I only hear 'my bonus'.
I suppose you could say 'my incentive bonus'.
Bu just 'my incentive' sounds wrong to me.
