Some people might use it as an expletive, eg. "What is the name of *** are you doing!!!!?"

It is, obviously, a religious expression and should not be used lightly. It could be interpreted as 'as *** is my witness' or 'I do this with the implied blessing of ***'. Many prayers in Christianity end with "..in the name of (***) the father, the son and the holy ghost." This means that the person is asking for the formal blessing on what has been prayed for.

I never use it or hear it. It seems to me very old-fashioned.

It sounds to me primarily like an expression of shock or surprise.

We do, however, commonly use the word '***', in casual English.
eg ***, that was a good movie!

Only when you are quoting Him from the Bible. Otherwise you are using His name in vein or swearing, neither of which is a good thing!

Never. I don't hear anybody else using it either.
