Can I say,

John saw a cat in /inside the drain.

John saw a cat was in the drain.

In the US, we would probably say "storm drain." These are conduits below and along the streets for rainwater runoff.
A cat would not fit in the drain in my sink or tub!

Fall / falling is OK. "into" is the best preposition.
