Hi,

Is there a case when in-house(adj) mean on the spot?

If yes, please give me examples

If not, how can we distinguish them?

I'd say it's very unlikely that in-house and on the spot could be used interchangeably . But, I'd also suggest you post a sentence (or sentences) in which you think the two might be interchangeable. book=Dictionary&va=in-house For additional dictionary help, look here
