In case they had or they were to have?

Hello everyone, I am having some trouble translating a sentence..in particular I can't find the difference between "in case they had and in case they were to have". Can anyone explain it to me please?

1. Miriam and Betty were sick. They were coughing and sneezing.  Everyone stayed away from the girls in case they had a bad virus that was contagious.   (A real possibility in the present.)

2. In case they were to have an accident, you must follow these instructions. (A hypothetical possibility in the future.)
