Just keep talking!

After having difficulty with a particular conversation, reconstruct it on paper. Write out all the things you tried to say, the things you wanted to say. Learn at your desk, while the pressure is off, how to say the things you wished you had been able to say during that conversation, using a dictionary and your grammar books. Try to recall what native speakers said during the conversation. You may want to borrow some of the expressions they used in talking about a particular topic of conversation. Memorize the word groups you will need most the next time this kind of conversation comes up. They might be single words, phrases of several words, or whole sentences. Practice saying these word groups as smoothly as possible. Try to combine your newly learned phrases in new ways. Caution: Be sure that you are saying them correctly by checking with a teacher, a textbook, or a forum like this one, because the last thing you want to do is memorize a phrase that is wrong!

By doing this consistently every day you will soon be able to speak fluently for longer and longer periods of time.
