I'm confusing the usage about "what does it mean" and "what is it meaning".

Would you please explain me what circumstance do we use those expression?

It is much the same as the difference between these two:

When do we meet?
When is the meeting?

The first uses a inflected (finite) verb form of "meet" and the second the non-finite form.
