I'm about to reply to an email regarding my request.

Dear Prof. (Name), I received a copy of my grade slip. Please find the attached PDF file below for your reference. Thank you for your assistance. With appreciation, STUDENT NAMESTUDENT ID
Correct the salutation if wrong.Also the 'Thank you for your assistance' part. And lastly, is the 'With appreciation,' correct? Someone correct it if I made a mistake. I am about to reply to email sent by a professor regarding my grade slip and requesting for shifting form as well.I'm not really good or fluent in writing emails in English I really struggled every time contacting them. Always trying my best but wasn't enough. I'm also confused if my name and student ID is necessary because I saw many samples on the internet that having continues conversation doesn't require it. As I said, I struggled.

Don't use "please find" unless the item has been lost. It is very annoying to me to be asked to find an email attachment, as if it was lost or I cannot see it. g.
