1. The underlined “they are doing” should be “they were doing” because the phrase, “If only they realized” is in past tense?

2. As written, (A) is an unfinished thought. It needs something to complete it, which might commonly begin "but also ...".

Both "are doing" and "were doing" are accepted there. "are" sounds a bit more vivid and immediate.
