A. is badly written, but it does not have the problems that B does.

That is better than A.

B: The Black Plague was spread by infected rats. A highly contagious disease, it was spread rapidly in the dirty, crowded medieval cities.

I forgot to add a comma after disease. Can it still be A then?? I am confused as to why it would be B rather than A. Can you please possibly explain? Thanks

It spread rapidly - that is better than the passive (it was spread rapidly) since there is no explicit agent.

The best version, of course, is the one not mentioned:

The Black Plague, a highly contagious disease, was rapidly spread by infected rats in the dirty, crowded medieval cities.

Coimbra19701A: The Black Plague
Your wording is muddled. It's The Black Death or The Plague.

That is a comparatively minor issue in this book, assuming that Coimbra has correctly written exactly what's in the book.
