I am an ex employee of MetLife. I had joined MetLife on 28/07/2008 and left on 9/04/2010.I was hired for FMLA(Family medical leaves act) and my id was 1036615.I had to resigned from MetLife due to personal family reason and this is the reason why I could not serve the notice period. I am aware of the fact that with clearing my dues, I won't get any kind of assistance to widraw my provident fund but due to some financial constraint I am unable to the quoted amount. I am trying to widraw my PF externally and for that I need a 3A form, if you could help me providing my
3A then that can resolves lot many problems for me.
