I really Need help

I m currently working for one of the reputed organizations in middle east and the organization where I m working is conservative and governmental in nature. I joined the organization only five months back and I wish to take up 5 days leave and wish to go back to my native place to see my parents as my father is not keeping good health.what shud be the format of the request letter to be there for this reason.I need somewhat not very straight forward mail Kindly do assist me.

On plain paper do I need to put title like this and put subject or something else to make little more elaborate.I m also putting my employee number at the end of an application below A.

Plain paper is fine; a letter needs no title. Yes, give your employee number under your name, and if it is a large organization, your job title and department.
