I need some help with a sentence

Hello,

Could you help me to rephrase this sentene please?

She had discharged herself against medical advice.

Is it possible to rephrase it in this way?- She was given official permission/exemption not to follow a medical advice.

I had trouble rewording this part; 'She had discharged herself'.
