I am gonna check their current HbA1c levels if they are in the target ranges. Target HbA1c levels vary with the patients' comorbid status, functional impairments, established diabetic complications, or limited life expectancies. I want to see how many patients are in the right range for their status.

I am thinking "Evaluation Of Geriatric Patients with Diabetes Mellitus According to Target HbA1c Levels" as title of my article but I am not sure if it is good enough.

Are going to plot distribution curves of the deviation from the targets per their comorbid status? It is not very clear what your research goals are, but the title should indicate that. The title that you have is not very illuminating.
