I have two questions.

1. If I use frequency words like always, often in a past tense sentence, does that sentence mean custom of the past?
eg. I often played tennis.

2. "How often have you been to America?" and "How many times have you been to America? " have the same meaning?
"Three" could be the answer of both sentence?
