I have a problem

Hi, people! I just want to ask you something about speaking English. I'm 14 years old and I'm studying English intesively, 18 hours a week. I'm really fond of the English language and I love learning it. The problem is that I still cannot speak English fluently. I mean I sometimes speak to myself, but I can't easily find the appropriate words in my head. My question is: How can I improve my speaking? I need your advice.
My other question is: How do I learn new words? I know that translating them into my language is not the best choice, is it?
