Hello guys, is it grammatical to say
"I dont know since when did I get this disease."?

or should it be
"I dont know since when I got this disease."?

Are there any better alternatives to say "I dont know since when..."?

Yes, definitely! You can say "I've had this disease since last summer," but "I don't know since when . . . " really doesn't work. Try one of these:

I don't know when I got this disease.
I don't know how long I've had this disease.
