In the States it's a vulgarism that means "angry", "irritated" or the like. I know that in some English speaking countries it means "intoxicated" but I'm not sure exactly which ones nor do I know how vulgar it is in such countries.

If you are pisst off, you are very angry.
In American English, you can leave "off" out, and you can say you are pisst if you are very angry. But in British English, if you leave "off" out and just say you are pisst, then it means you are drunk. (the word "pisst" has been misspelled on purpose).
